Summary
Precarious work characterised by low pay, inadequate and variable hours, and short-term/temporary contracts refers to the employment conditions that include the situations of uncertainty’, ‘instability’, and ‘insecurity’. Precarious work arrangements skyrocketed in the 1970s as a result of the worldwide recession and the weakened power and forced reduction of organised labour. As a result, considerable power shifted back to employers who steadily repeal employment protections whenever possible. This chapter seeks to shed light on the missing link between diversity and precarious work from the viewpoint of sexual and gender identity minorities whose voices have been silenced and marginalised in mainstream discussions. Due to the long history of legalised discrimination and the stigmatisation they experience, LGBTQ+ employees are overrepresented in precarious work arrangements. Since LGBTQ+ employees are already a vulnerable population, the adverse effects of precarious work arrangements are exacerbated. We adopt an institutional approach to explore the challenges LGBTQ+ individuals face in precarious work concerning macro-institutional factors (e.g., social, organisational, political, legal, and economic) and micro-level factors (e.g., interpersonal discrimination, exclusion, incivility within organisations). Through case studies from Turkey and the United States, we demonstrate the political and social mechanisms that legitimise and proliferate precarious work arrangements for LGBTQ+ individuals. We conclude the chapter by highlighting research questions that scholars should pursue further to uncover the unique challenges LGBTQ+ individuals endure in precarious work arrangements.

Objective:
A standardised multi-site approach to manage paediatric post-operative chylothorax does not exist and leads to unnecessary practice variation. The Chylothorax Work Group utilised the Pediatric Critical Care Consortium infrastructure to address this gap.
Methods:Over 60 multi-disciplinary providers representing 22 centres convened virtually as a quality initiative to develop an algorithm to manage paediatric post-operative chylothorax. Agreement was objectively quantified for each recommendation in the algorithm by utilising an anonymous survey. “Consensus” was defined as ≥ 80% of responses as “agree” or “strongly agree” to a recommendation. In order to determine if the algorithm recommendations would be correctly interpreted in the clinical environment, we developed ex vivo simulations and surveyed patients who developed the algorithm and patients who did not.
Results:The algorithm is intended for all children (<18 years of age) within 30 days of cardiac surgery. It contains rationale for 11 central chylothorax management recommendations; diagnostic criteria and evaluation, trial of fat-modified diet, stratification by volume of daily output, timing of first-line medical therapy for “low” and “high” volume patients, and timing and duration of fat-modified diet. All recommendations achieved “consensus” (agreement >80%) by the workgroup (range 81–100%). Ex vivo simulations demonstrated good understanding by developers (range 94–100%) and non-developers (73%–100%).
Conclusions:The quality improvement effort represents the first multi-site algorithm for the management of paediatric post-operative chylothorax. The algorithm includes transparent and objective measures of agreement and understanding. Agreement to the algorithm recommendations was >80%, and overall understanding was 94%.

The study of microfoundations, especially individuals, is enjoying a renaissance in international relations (IR) scholarship. Yet, this rise is more difficult to find in publication data. Using the Teaching, Research, and International Policy (TRIP) journal-article database, we show that only 13.7% of IR articles in 12 leading journals use the first image. This proportion remains approximately the same from 1980 through 2018. Interrogating the data, we show that this distribution does not stem from epistemological or methodological commitments, such as positivism, quantitative analysis, or formal modeling. We suggest several reasons for this apparent disjuncture between qualitative assessments of the rebirth of first-image theorizing and the quantitative data that imply a slower or perhaps more limited return.

In much of Europe, the advent of low-input cereal farming regimes between c.ad 800 and 1200 enabled landowners—lords—to amass wealth by greatly expanding the amount of land under cultivation and exploiting the labour of others. Scientific analysis of plant remains and animal bones from archaeological contexts is generating the first direct evidence for the development of such low-input regimes. This article outlines the methods used by the FeedSax project to resolve key questions regarding the ‘cerealization’ of the medieval countryside and presents preliminary results using the town of Stafford as a worked example. These indicate an increase in the scale of cultivation in the Mid-Saxon period, while the Late Saxon period saw a shift to a low-input cultivation regime and probably an expansion onto heavier soils. Crop rotation appears to have been practised from at least the mid-tenth century.

We propose the concept of the “Fish Revolution” to demarcate the dramatic increase in North Atlantic fisheries after AD 1500, which led to a 15-fold increase of cod (Gadus morhua) catch volumes and likely a tripling of fish protein to the European market. We consider three key questions: (1) What were the environmental parameters of the Fish Revolution? (2) What were the globalising effects of the Fish Revolution? (3) What were the consequences of the Fish Revolution for fishing communities? While these questions would have been considered unknowable a decade or two ago, methodological developments in marine environmental history and historical ecology have moved information about both supply and demand into the realm of the discernible. Although much research remains to be done, we conclude that this was a major event in the history of resource extraction from the sea, mediated by forces of climate change and globalisation, and is likely to provide a fruitful agenda for future multidisciplinary research.

The early Middle Ages saw a major expansion of cereal cultivation across large parts of Europe thanks to the spread of open-field farming. A major project to trace this expansion in England by deploying a range of scientific methods is generating direct evidence for this so-called ‘Medieval Agricultural Revolution’.

In the Dahomeyan of the western Accra Plains mafie gneisses, amphibolites, and ortho-amphibolite dykes in granitic gneiss occur. During the latest recrystallization, metamorphic conditions ranged (north to south) from oligoclase-epidote amphibolite subfacies through andesine-epidote amphibolite subfacies to at least the medium or upper part of the amphibolite facies. Chemical analyses of hornblende, garnet, and clinopyroxene from two mafic gneiss specimens supplement data from the Mampong specimen (Knorring and Kennedy, 1958) and indicate that mineral compositions are largely controlled by rock compositions.

Summary
Introduction
Despite a decrease in the mortality rates of sudden infant death syndrome (SIDS) over the past decades, SIDS is still one of the leading causes of post-neonatal and infant death. In the United States, 8% of all infant deaths are attributed to SIDS, only behind congenital malformations and chromosomal abnormalities (21%) and disorders relating to prematurity and low birth weight (17%) (1). While efforts have been made to reduce the role of environmental factors, such as sleep environment and smoking, this persistent mortality highlights the importance of intrinsic factors involved in SIDS, including genetic changes that predispose infants to, or are directly responsible for, SIDS (2, 3). In addition, the incidence of SIDS in families where one infant has died from SIDS is increased by over fivefold, providing further evidence of a role for genetic factors (4, 5).
A role for genetic factors in SIDS is consistent with the Triple Risk Model of SIDS, with genetic factors contributing to the “vulnerable infant”. The Triple Risk Model hypothesizes that three elements are present for SIDS to occur (6):
1. Critical developmental period: Mortality from SIDS clusters during certain ages. Developmental changes in autonomic control co-incide with periods of increased susceptibility to SIDS.
2. Vulnerable infant: Understood largely in terms of associated risk factors such as race or exposure to alcohol or tobacco during pregnancy; infants dying from SIDS have been shown to have differences in autonomic responses that impair their ability to respond to challenges in the sleep environment. In addition, genetic conditions, such as a cardiac channelopathy, a metabolic condition, or a seizure disorder, may cause the infant to be vulnerable and at greater risk of SIDS.
3. Exogenous stressor(s): Stressors associated with SIDS include overheating, secondhand tobacco smoke, upper respiratory tract infection, bed sharing, and prone sleeping position (6).
According to the Triple Risk Model, although most babies encounter and survive environmental stressors, a vulnerable infant who becomes challenged during a critical period may not be able to overcome the stressor, leading to SIDS. Thus, SIDS can be seen as representing a severe, lethal phenotype with genetic causes that contribute to the vulnerable infant. Genomic approaches to SIDS attempt to understand genetic mechanisms causing or contributing to this infant vulnerability.

The use of underground geological repositories, such as in radioactive waste disposal (RWD) and in carbon capture (widely known as Carbon Capture and Storage; CCS), constitutes a key environmental priority for the 21st century. Based on the identification of key scientific questions relating to the geophysics, geochemistry and geobiology of geodisposal of wastes, this paper describes the possibility of technology transfer from high-technology areas of the space exploration sector, including astrobiology, planetary sciences, astronomy, and also particle and nuclear physics, into geodisposal. Synergies exist between high technology used in the space sector and in the characterization of underground environments such as repositories, because of common objectives with respect to instrument miniaturization, low power requirements, durability under extreme conditions (in temperature and mechanical loads) and operation in remote or otherwise difficult to access environments.

Speleothem carbon and oxygen isotopic records from Onondaga Cave, south-central Missouri, and Beckham Creek Cave, north-central Arkansas, are compared with the Cupola Pond and Oldfield Swamp pollen series from southeastern Missouri and the Rodgers Shelter and Modoc Shelter vertebrate biostratigraphic sequences from central Missouri and southwestern Illinois. Similar, and roughly contemporaneous, shifts between deciduous forest and steppe indicators throughout the Holocene are revealed in each database. These independent proxies record steppe conditions between approximately 9000 and 1500 cal yr B.P. A shift toward lighter speleothem carbon may reflect a change from warm and dry to cool and dry conditions between 4500 and 3000 yr B.P. The sensitive response of speleothem δ13C to changes in vegetation emphasizes their importance as paleoclimate records in an area containing few other millenial-scale climate proxies.

Objective
To examine the use of vitamin D supplements during infancy among the participants in an international infant feeding trial.
DesignLongitudinal study.
SettingInformation about vitamin D supplementation was collected through a validated FFQ at the age of 2 weeks and monthly between the ages of 1 month and 6 months.
SubjectsInfants (n 2159) with a biological family member affected by type 1 diabetes and with increased human leucocyte antigen-conferred susceptibility to type 1 diabetes from twelve European countries, the USA, Canada and Australia.
ResultsDaily use of vitamin D supplements was common during the first 6 months of life in Northern and Central Europe (>80 % of the infants), with somewhat lower rates observed in Southern Europe (>60 %). In Canada, vitamin D supplementation was more common among exclusively breast-fed than other infants (e.g. 71 % v. 44 % at 6 months of age). Less than 2 % of infants in the USA and Australia received any vitamin D supplementation. Higher gestational age, older maternal age and longer maternal education were study-wide associated with greater use of vitamin D supplements.
ConclusionsMost of the infants received vitamin D supplements during the first 6 months of life in the European countries, whereas in Canada only half and in the USA and Australia very few were given supplementation.

Objective: The objective of this study was to assess the impact of the 2010 Chilean earthquake on hospital functions and services. Hospitals functioning in a post-disaster environment must provide emergency medical care related to the event, in addition to providing standard community health services. This study focused on damage to both structural and nonstructural components, as well as to utility services.
Methods: Site visits were made to every hospital in a single province (Bio-Bio). Engineers conducted damage assessments while interviews of hospital administrators were conducted. The survey was requested by the Chilean Ministry of Health (MOH) to assess the impact of the earthquake on hospital operations and facility responses to those effects. Other important regional and hospital data were gathered from hospital administrators and the MOH.
Results: Seven government hospitals were surveyed. All hospitals in the region lost communications, municipal electrical power and water for several days. All reported some physical damage although only one suffered significant structural damage. All lost some functional capacity as a result of the earthquake. The loss of telephones and cellular service was identified as the most difficult problem by administrators. An average of 3 physical areas per hospital lost some degree of functional capacity following the earthquake.
Conclusion: Even in an earthquake-prone and very well-prepared country such as Chile hospital functions were widely disrupted by the event. The loss of hospital functions can occur even with minimal damage to the physical structure. The loss of communications can impede or halt response efforts at all levels. Hospitals should be prepared to self-sustain following a disaster for 2-3 days regardless of the level of structural damage. Understanding the details of these impacts is essential to hospital preparedness and plans for continuing services after a disaster.

Systemic corticosteroids are a mainstay of treatment for many pediatric medical conditions. Although their impact on the central nervous system has been well-studied in animal models and adults, less is known about such effects in pediatric populations. The current study investigated acute effects of corticosteroids on memory, executive functions, emotion, and behavior in children and adolescents with inflammatory bowel disease (IBD). Patients 8–17 years with IBD (Crohn's disease, CD; ulcerative colitis, UC) on high-dose prednisone (n = 33) and IBD patients in remission off steroids (n = 33) completed standardized neuropsychological tests and behavior rating scales. In the IBD sample as a whole, few steroid effects were found for laboratory cognitive measures, but steroid-treated patients were rated as exhibiting more problems with emotional, and to a lesser extent with cognitive function in daily life. Steroid effects, assessed by laboratory measures and questionnaires, were more prevalent in CD than UC patients; UC patients on steroids sometimes performed better than controls. Sleep disruption also predicted some outcomes, diminishing somewhat the magnitude of the steroid effects. Corticosteroid therapy can have acute effects on cognition, emotion, and behavior in chronically ill children; the clinical and long-term significance of these effects require further investigation.
